    1.00 amputation. Injured by slipping or swinging object held by injured worker — Amputations — Finger(s), fingernail(s), n.e.c.. An employee was cutting a 4'x8' sheet of OSB with a saw when the saw's cord caught on the bottom of the sheet. The saw kicked off the board and contacted the employee's left hand, resulting in the amputation of three fingers. The middle finger was reattached and the other two fingers required stiches.
